The controversy of Form 15 can be resolved if we look at it as a record of consumption of ballot papers dumped into the bags as distinct from a statement submitted by an official to his supervisor which together with other criteria constitutes the basis of his performance evaluation.
This explains the deficit shown in the exercise of duty of care and diligence on the part of electoral officials.
